Introduction
This Telehealth Informed Consent explains how care may be delivered through remote and asynchronous methods when you participate in Eterna IQ membership and related clinical services. It is incorporated into our Terms of Service.
Eterna IQ, Inc. ("Eterna IQ") is a health technology company. Eterna IQ does not practice medicine. Clinical services are provided by independent, licensed clinicians affiliated with partner medical groups such as [CAREVALIDATE AFFILIATED PC LEGAL NAME] (the "Medical Group").
What telehealth includes
Telehealth may include secure messaging, asynchronous review of health questionnaires and laboratory results, video visits when clinically appropriate, and coordination with coaches and fulfillment partners.
Your care may begin with an online intake or assessment. A licensed provider reviews your information and determines whether telehealth is appropriate and whether any in-person evaluation is needed.
Benefits and limitations
Telehealth can improve access to longitudinal care, coaching, and follow-up. It may reduce travel and wait times.
Telehealth has limitations. A provider cannot perform a full physical examination remotely. Technical failures, incomplete information, or emergency symptoms may require in-person care or emergency services.
Independent clinical judgment
Licensed clinicians exercise independent medical judgment. Eterna IQ does not direct prescribing, dosing, or treatment decisions.
Your treating relationship for medical care is with the Medical Group and its clinicians, not with Eterna IQ.
No guarantee of treatment or outcomes
Completion of an assessment, quiz, or intake does not guarantee a prescription, laboratory order, or specific therapy.
A prescription is issued only when a licensed clinician determines it is clinically appropriate. If no prescription is issued where one is required, the related order may be declined and refunded per our Refund Policy.
Individual results vary. Eterna IQ does not guarantee specific health outcomes.
Compounded prescription therapies
Some therapies available through membership may be compounded by licensed partner pharmacies when clinically appropriate.
Compounded drug products are not FDA-approved in the same manner as commercially available FDA-approved drugs unless specifically stated. The FDA does not review compounded drugs for safety, effectiveness, or quality before they are marketed.
You acknowledge that you have been offered our Important Safety Information before starting therapy in a pathway that includes prescription treatment.
Your responsibilities
You agree to provide accurate and complete health information, including medications, allergies, pregnancy status, and relevant medical history.
You agree to follow your clinician's instructions, attend recommended follow-up, and promptly report side effects or changes in your condition.
Eterna IQ is not liable for outcomes resulting from inaccurate, incomplete, or misleading information you provide.
Not for emergencies
Telehealth services and Eterna IQ membership are not for medical emergencies. If you think you may have a medical emergency, call 911 or go to the nearest emergency room immediately.
Privacy and medical records
Medical records created by the Medical Group and its clinicians are maintained by those covered entities under their privacy practices. Eterna IQ facilitates access to partners but is not your treating provider.

Consent and withdrawal
By using clinical services facilitated through Eterna IQ, you consent to receive care via telehealth methods described here.
You may withdraw this consent at any time by notifying your clinician or emailing support. Withdrawal may require discontinuing certain therapies or services that cannot be delivered safely without telehealth.
